Removable implant - borne reconstruction

Introduction

This is a challenging case.

The patient has lost many upper teeth. She presented for treatment wearing a plastic removable partial denture. She was seeking an implant solution. 

Plastic Partial Denture

This removable plastic partial denture was constructed  to replace the missing teeth (6951).

 

The partial denture was unsightly and very difficult to wear.

A look inside the mouth shows a  how the loss of teeth had resulted in shrinkage of the bone and drifting together of the remaining two front teeth.

The Result

Would it  be feasible to offer the patient an implant solution?

 

The answer is yes!

 

We presented her with a treatment plan and explained in detail the challenges that needed to be addressed. 

 

She excited to go ahead.

Case Walkthrough

Let’s walk you through the case and understand how we did it.

We were able to achieve the placement of four strategically positioned implants having created adequate bone to accommodate these implants.

 

We  extracted the remaining two front teeth and fitted  a temporary partial denture which enable us  to proceed with the final restorative solution. This partial denture was worn whilst implants integrated to the bone. Once integration  of the implants had taken place, the final restorative treatment began

 

Special Attachments

Special attachments were fabricated in Spain and sent back to South Africa.

 

These attachments compensated for the slightly misplaced implants and provide the connection for  the final restorative implant borne partial denture.

This partial denture is delicate in design. It has a firm connection with the special attachments and is removable to facilitate effective oral hygiene.

It was decided not to  prescribe  a  fixed hybrid denture.

The  correct placement of  the implants is critical and very difficult to achieve. 

 

A hybrid  fixed denture does not allow for effective oral hygiene and the long-term health of implants is compromised.

 

Achieving effective effective phonetics and aesthetics

From start to finish, the treatment extended over 11 months.

 

During all this time the patient was kept comfortable by wearing a provisional denture.

 

This allowed her to continue to look good while implants were healing and then made it easy the various treatment stages to be done.

 

Whilst in treatment , nobody knew from her outward appearance that  major dentistry was in progress.

 

We have provided the patient with a successful long-term solution.
